- A medical invoice lists each service with its date, a plain description and its charge, then shows payments received and the balance due.
- In the UK, HMRC treats care aimed at protecting or restoring health as exempt from VAT, but most reports and certificates are standard-rated, so one invoice can carry both.
- German private doctors must itemize by fee number and multiplier under GOÄ section 12, and US self-pay patients can dispute a bill that is at least $400 over their good faith estimate.
A medical invoice is a bill from a doctor, clinic or therapist to a patient who pays for care directly, listing each service, its date and its charge, with any payment received and the balance due. A medical invoice template needs the same basics as any invoice, plus a few items that patients and payers look for: the date of each service, a plain description, and a clear balance.
This guide gives you a free medical invoice template with a filled-in US self-pay example, then the country rules that change what the invoice must say: VAT-exempt care and taxable reports in the UK, and the itemized fee-schedule invoice that German doctors issue. Claims to health insurers follow the insurer's own claim process, so this guide covers invoices to patients and to businesses that pay for care.
What must a medical invoice include?
A medical invoice must tell the patient what they are paying for and why the amount is what it is. A self-pay patient is a patient who has no insurance or chooses not to use it, and who is billed directly. Use these fields:
| Field | Example | Why it matters |
|---|---|---|
| Practice name, address, phone | Riverside Family Clinic, Portland, OR | Identifies the provider |
| Patient name | Jordan Ellis | Who is billed |
| Invoice number and date | MED-2026-0412, October 14, 2026 | Unique and sequential |
| Date of each service | October 12, 2026 | Patients and payers check this |
| Service description | "Office visit, established patient" | Plain words, not shorthand only |
| Charge per service | $165.00 | One line each |
| Payments and discounts | Paid at visit $100.00 | Shows how the balance arises |
| Balance due and due date | $128.00, due November 13, 2026 | Gets you paid |
| Billing contact | Phone and email for questions | Reduces disputes |
Include only what the reader needs. A bill that goes to an employer or a relative should not carry more clinical detail than the payer must see.
Medical invoice template, filled in: a US self-pay visit
This medical invoice template bills a self-pay patient for an office visit, a lab panel and an injection. The patient paid part at the visit, so the invoice shows the balance.
| Date | Description | Amount |
|---|---|---|
| October 12, 2026 | Office visit, established patient | $165.00 |
| October 12, 2026 | Basic metabolic panel (lab) | $38.00 |
| October 12, 2026 | Injection administration | $25.00 |
| Invoice total | $228.00 | |
| Paid at visit on October 12, 2026 | −$100.00 | |
| Balance due | $128.00 |
Patients who pay out of pocket have a right to an estimate first. The Centers for Medicare & Medicaid Services (CMS) says that if you do not have or use health insurance, providers usually must give a good faith estimate of what care will cost. CMS adds that they must provide it when you ask or when you schedule services at least 3 business days in advance, according to its good faith estimate page.
The same page says a patient may be able to dispute a bill that is at least $400 more than the estimate. In the example, the estimate was $240.00 and the invoice is $228.00, so the bill came in under it. A $700.00 bill against the same estimate would be $460.00 over, past the $400 line.

Medical invoice in the UK: exempt care and taxable reports
A UK medical invoice shows VAT only on the parts that are taxable, and most clinical care is not. HMRC's VAT Notice 701/57 on health professionals says services are exempt when the primary purpose is the protection, maintenance or restoration of the person's health. The same notice says most supplies of certificates and reports are standard-rated, and it lists medico-legal reports, court evidence and pre-employment medicals as taxable.
The standard VAT rate is 20%, according to GOV.UK's VAT rates page. So a private clinic often issues one invoice with an exempt line and a taxable line:
| Description | Net | VAT | Total |
|---|---|---|---|
| Private consultation, October 12, 2026 (exempt) | £120.00 | £0.00 | £120.00 |
| Medico-legal report for a court case (standard rate) | £250.00 | £50.00 | £300.00 |
| Invoice total | £370.00 | £50.00 | £420.00 |
Mark each line "exempt" or "standard rate" so the patient can see why only one line carries VAT. Contact HMRC or an accountant for your clinic's own position, because the answer depends on its mix of income.
Medical invoice in Germany: the Rechnung under the GOÄ
German private doctors bill under a national fee schedule, and the invoice must show it line by line. Section 12(2) of the Gebührenordnung für Ärzte (GOÄ) requires the invoice to show the date of each service, the fee number and description, the amount and the multiplier. If a fee exceeds 2.3 times the base rate, the doctor must give a written reason for that service.
The Gebührenordnung für Ärzte (GOÄ) is the fee schedule that sets what private patients are billed for medical services in Germany. Each service has a number and a point value, and the doctor multiplies the base fee by a factor. A basic consultation, number 1, has 80 points in the GOÄ fee list, which converts to roughly €4.66 at the single rate.
An example invoice at the 2.3 multiplier:
| Date | GOÄ number | Description | Factor | Amount |
|---|---|---|---|---|
| October 12, 2026 | 1 | Beratung (consultation) | 2.3 | €10.72 |
| October 12, 2026 | 5 | Symptombezogene Untersuchung (symptom-focused examination) | 2.3 | €10.72 |
| Total | €21.44 |
Medical treatment itself is VAT-exempt. Section 4 no. 14(a) of the Umsatzsteuergesetz (UStG) exempts treatments in human medicine carried out by doctors, dentists, alternative practitioners, physiotherapists, midwives or similar practitioners. The invoice therefore shows no VAT, and a note that the service is exempt is enough.
Always check the current numbers and factors against the official fee schedule before issuing real invoices. The table above shows the layout, not a billing recommendation.
Medical invoice to an employer or other third-party payer
Some medical invoices go to a company, not a patient. Occupational health checks, pre-employment medicals and fitness assessments are typical. A third-party payer is any organization, other than the patient, that pays for the care. The invoice then names the company as the customer and the worker as the person examined.
For example, a UK occupational health practice bills a company for a pre-employment medical. HMRC's VAT Notice 701/57 lists pre-employment medicals as taxable, because they are mainly for the employer's decision. The invoice looks like this:
| Description | Net | VAT | Total |
|---|---|---|---|
| Pre-employment medical, candidate ref. 2291, October 12, 2026 | £180.00 | £36.00 | £216.00 |
Use a reference, not a diagnosis. The company needs to match the invoice to a purchase order or candidate, and it does not need clinical detail. Put the company's purchase order number on the invoice if it gave you one, and use payment terms that match the contract.
What changed in 2026: guidance dates to check
Rules on medical billing change slowly, so the news is mostly about checking dates. HMRC's page for VAT Notice 701/57 shows a last update of August 13, 2026. If your clinic issues mixed exempt and taxable invoices, read the current notice instead of relying on an older copy.
In Germany, the Federal Ministry of Finance says in its E-Rechnung questions and answers that other invoice formats may be used until December 31, 2026, or until December 31, 2027 for issuers with prior-year turnover of €800,000 or less. Invoices to private patients are not business-to-business. A practice that also bills companies, for example for occupational health, should ask its tax adviser whether that part of its work is in scope.
In the US, the good faith estimate rules for self-pay patients are in force, and the $400 dispute line above is the number patients are told to watch. A practice that gives estimates should keep them with the patient file, so the invoice can reference the date and amount.
How to word payment plans and late balances
A medical invoice often leads to a payment plan, and the invoice should say so in plain terms. For example, if the patient owes $128.00 and agrees to pay $64.00 on November 13 and $64.00 on December 13, 2026, write that on the invoice and in an email. A written plan prevents a "final notice" arriving for money that is already scheduled.
Keep the tone neutral. State the balance, the dates and the way to pay. Leave out words that suggest blame, because patients often delay payment from confusion, not refusal.
Common mistakes on a medical invoice
Medical invoices are disputed for predictable reasons:
- Vague descriptions. "Services" gives the patient nothing to check.
- No service dates. Patients compare the bill with their calendar.
- Missing payments. A deposit or copay not shown makes the balance look wrong.
- No estimate reference. Self-pay patients compare the bill with the estimate.
- Too much clinical detail. Share only what the payer needs.
- Mixed tax treatment unmarked. In the UK, label exempt and standard-rate lines.
Our guide Invoice email template: how to email an invoice has wording for the message that carries the PDF, and What is the difference between an invoice and a receipt? explains what to send once the patient has paid.
Final check before you send a medical invoice
Before you send, check these six points:
- Each service has a date, a plain description and a charge.
- Payments already received are shown as separate lines.
- The balance, due date and billing contact are on the page.
- For self-pay patients, the estimate is referenced.
- Tax treatment, exempt or taxable, is right for your country.
- Nothing sensitive appears that the reader does not need.
Frequently asked questions
What should a medical invoice include?
A medical invoice should include the practice's details, the patient's name, an invoice number and date, the date and plain description of each service, the charge per service, payments received, the balance due, the due date and a billing contact.
Is a medical invoice the same as an insurance claim?
No. A medical invoice bills the patient or another payer directly. A claim to a health insurer follows the insurer's own claim process and forms. Self-pay patients and businesses that pay for care are the usual recipients of an invoice.
Do I charge VAT on a private medical consultation in the UK?
Usually not. HMRC's VAT Notice 701/57 treats services whose primary purpose is protecting, maintaining or restoring health as exempt, though most certificates and reports are standard-rated. Label each line on the invoice as exempt or standard rate, so the patient can see why.
What is a good faith estimate for self-pay patients?
It is a written estimate of expected charges that providers usually must give patients who have no insurance or do not use it. According to CMS, a patient may be able to dispute a bill that is at least $400 more than the estimate.
